Laser eye surgery in Turkey

All laser vision correction does the same thing: an excimer or femtosecond laser removes a precise layer of corneal tissue so that the cornea's curve matches your prescription. The procedures differ in how the laser reaches that tissue. LASIK cuts a thin flap, lifts it, reshapes underneath and lays it back. SMILE removes a small lenticule of tissue through a keyhole incision with no flap. PRK and its variants remove the surface layer first and let it regrow. Each has a candidate profile, a recovery pattern and a price, and the examination on arrival decides between them, not the brochure.

Why Turkey: volume and price. The large eye hospitals grew up on laser correction for Turkish patients long before international ones arrived, so the surgeons who operate on you are doing the same operation many times a week, and the platforms, femtosecond lasers for the flap, SMILE systems, wavefront-guided excimer lasers, are the ones sold everywhere else. The price is usually quoted for both eyes and often as a package with the hotel and transfers, which is the number people compare with a per-eye private quote at home. Most patients come from the UK and Ireland, Germany and the Gulf, and most of them go to Istanbul; Antalya has a smaller cluster of established clinics and is where the beach-and-surgery packages come from.

What the written quote must name before you pay:

• Whether the price is for both eyes, and what changes if only one eye is suitable or the examination changes the procedure
• Whether you pay for the examination if you turn out not to be a candidate
• The enhancement policy: what is covered, for how long, and whether it requires a second trip
• The drops and the number of check-ups during your stay
• Who treats a complication in the first weeks, where, and at whose cost
• Transfers, hotel nights and interpreter, if the price is presented as a package, and the surgeon's name

Searching for which laser eye clinic in Turkey is the one to pick brings up pages that award themselves the title; nobody keeps that ranking. What you can check is more useful. The clinic must be licensed by the Turkish Ministry of Health. The surgeon must be an ophthalmologist whose work is cornea and refractive surgery, not a general eye surgeon who lasers on Fridays; ask how many laser procedures they do in a year and how many of the one you are having. Ask which platform they use for the flap and for the reshaping, and whether SMILE and PRK are also on the table, because a clinic with all three chooses; a clinic with one sells. Ask who sees you the next day and who answers the phone once you are home. An international hospital accreditation such as JCI describes the hospital's processes, not the surgeon's hands.

Istanbul has the most eye hospitals, the most international departments and the most flights, so most patients go there. Antalya has a smaller group of established clinics and a resort around them, and the packages that combine surgery with a hotel week come from there; the surgery is the same, and the choice should still be made on the surgeon. Izmir and Ankara have good clinics with fewer foreign patients. Whichever city, do not book a holiday-first itinerary: no swimming, no sunbathing with sweat running into the eyes and no dusty excursions in the days after surgery.

After LASIK and SMILE, most people see well the next day and are back at a screen within days; the drops continue on the written schedule, the eyes stay dry and light-sensitive for a while, and the prescription settles over months. Keep water, soap and make-up away from the eyes for the first week, do not rub them, sleep with the shields as long as you are told, and wait for the surgeon's go-ahead before swimming, contact sports and dusty work. After PRK, count on several uncomfortable days and a fortnight of blur before the surface has healed and the drops schedule can ease.

What happens years later is the question people ask least and should ask most. The reshaped cornea is stable in most people, and a small regression, usually in the higher prescriptions, is what enhancements are for. Presbyopia arrives regardless: in your forties or fifties you will need reading glasses whether or not you had laser, because that is the lens ageing, not the cornea. A cataract can still form later, and the surgeon who operates on it will need your laser records to calculate the new lens, which is one more reason to keep the operation report. Dry eye that persists is the long-term complaint that matters most, and it is more likely if your tear film was marginal before surgery, which is a screening question, not luck.
