Breast lift with teardrop implants in Turkey

A breast lift with implants is two operations performed at once, and they pull in opposite directions. The lift removes skin and tightens the envelope; the implant fills and stretches it. Everything difficult about this operation, and the reason it is revised more often than either the lift or the augmentation done alone, comes from balancing those two forces on the same breast.

The teardrop part refers only to the implant's shape. Anatomical implants are fuller at the bottom than the top, and they have to stay in the right orientation to look right, which is a different consideration from whether you need a lift at all. That decision is worth taking separately, and it starts with what is actually bothering you: position, or volume, or both.

Which problem are you actually solving

There are three distinct complaints and they have three different answers. If the breast has lost volume but sits where you want it, an implant alone may be enough. If the volume is fine but the breast and nipple sit lower than you want, a lift alone is the operation. If both are true, the combined procedure is what is being discussed here.

Ask the surgeon to say which of the three you are, and to say it before any implant catalogue comes out. The order of that conversation tells you a great deal: an assessment that begins with sizes has started at the wrong end.

  • Volume lost, position fine

    An implant alone may be enough, and adding a lift adds scars for nothing. This is the case people most often over-treat.
  • Position dropped, volume fine

    A lift alone is the operation, and it leaves the breast the size it is. Adding an implant here is a decision about size and not about lift.
  • Both, after pregnancy or weight loss

    The combined operation. It is the more demanding plan and the one with the higher revision rate, and it is worth knowing that before you book.
  • Still changing

    If you are still losing weight or plan another pregnancy, both the skin and the volume will change again. Operating now is buying a revision later.

One operation or two

Doing the lift and the augmentation in one sitting means one anaesthetic, one recovery and one bill. Doing them in stages, usually the lift first and the implant later, means the surgeon can see how the tissue settled before deciding how much to fill it, and it separates two sets of forces that would otherwise be applied to the same skin on the same day.

Neither approach is right for everyone. What matters is that you are told which is being proposed for you and why, in terms of your tissue rather than the schedule. A surgeon who only ever offers one-stage surgery is describing a preference, not an assessment, and staging is not a failure or an upsell.

What the teardrop shape does and does not change

An anatomical implant is fuller at the lower pole, which is meant to produce a more sloped upper chest. The trade is that it has to stay in the right orientation: if an anatomical implant rotates, the breast looks wrong and correcting it means another operation. A round implant cannot rotate in a way anyone can see, because it is the same shape all the way round.

The other thing worth knowing is that most anatomical implants are textured, because texture is what holds them in position. Textured surfaces are the ones associated with breast implant-associated anaplastic large cell lymphoma, and certain macro-textured devices were withdrawn from the market worldwide after regulators in Europe, North America and Australia acted. That is a question to ask about the specific device you are being offered, by name.

How the operation works

  1. Assessment before sizing

    Measurements, skin quality, nipple position and how much tissue you have, discussed before any implant is chosen. Ask which of the three problems you have.
  2. Choosing the implant and the plane

    Shape, size, surface and whether it sits above or below the muscle. Ask for the manufacturer and model in writing, and keep the implant card.
  3. Marking

    Done with you standing, because the breast is a different shape lying down. This is where the scar pattern is decided and shown to you.
  4. The lift

    Skin is removed and the nipple is moved to its new position on its own blood supply. The scar pattern follows how much lift is needed, not what you would prefer.
  5. The implant

    Placed through the same incisions, in the pocket planned. With an anatomical device the pocket has to fit closely, because a loose pocket is what lets it rotate.
  6. Weeks, then a year

    Support garment, no lifting or upper body training, and a shape that keeps changing. Implants settle and scars mature over about a year, so judge the result then.

The risks worth understanding

  • Higher revision rate than either operation alone. This is the defining fact of the combined procedure and it follows from the two forces working against each other.
  • Blood supply to the nipple. A lift moves the nipple on its own blood supply and an implant increases the tension around it. Combining them is precisely why this risk is discussed more here than for a lift alone.
  • Sensation change. Reduced or altered nipple sensation is a recognised outcome and is not always temporary.
  • Capsular contracture. Scar tissue tightening around the implant, which can change the shape and cause discomfort and is a common reason for further surgery.
  • Rotation, with anatomical implants. The specific failure mode of the teardrop shape, and correcting it means returning to theatre.
  • Wound healing where the scars meet. The junction of the vertical and horizontal scars is the spot most likely to be slow to heal, and smoking makes it considerably worse.
  • Implants are not lifetime devices. Further surgery at some point over a lifetime is expected rather than exceptional, and that belongs in the cost of the decision.

Who this operation does not suit

  • Anyone still losing weight or planning a pregnancy. Both change the skin and the volume again, and this is the operation least tolerant of that.
  • Anyone who smokes and will not stop. Wound healing at the scar junction is the weak point and smoking is the largest thing you control.
  • Anyone who wants a large size and a tight lift at once. These pull against each other. You can have a compromise between them; you cannot have both at their maximum.
  • Anyone unwilling to accept lift scars. There is no version of a lift without them, and an implant alone will not achieve the same position.
  • Anyone who has not been assessed standing. A plan made from photographs has not measured the thing that decides the scar pattern.

Having it done in Turkey

  • Ask for the implant details by name. Manufacturer, model, surface, size and the implant card to take home. Without it no one can help you years later.
  • Ask about the surface specifically. Whether the device is textured or smooth, and what the surgeon's position is on textured devices given the lymphoma association.
  • Ask whether one stage or two is being proposed, and why. In terms of your tissue, not the length of your trip.
  • Ask to see healed results at a year. Photographs at a few weeks show a shape that has not settled and scars that have not matured.
  • Ask what a revision costs and who pays. This operation is revised more often than most, and a package that ends at the airport covers none of it.
  • Agree the follow-up in writing. Names, dates and a route back to the surgeon from home for at least the first year.

What sets the price in Turkey

  • The implants themselves. Manufacturer, shape and surface all move the price, and anatomical devices are usually priced above round ones.
  • One stage or two. Two operations mean two anaesthetics and two recoveries, which is a different product and should be quoted as one.
  • Theatre time and anaesthetic. The combined procedure takes longer than either operation alone.
  • Hospital or clinic and length of stay. Day case and an overnight stay are not the same thing, and the paperwork should say which you are buying.
  • Garments and aftercare. Support bra, dressings, suture removal and how many reviews are covered before you fly home.
  • What a revision would cost. Ask now, because it is the most likely additional cost of this particular operation.

Do I need a lift as well as implants?
It depends on where the breast and nipple sit rather than on volume. If the tissue sits low, an implant alone fills it without raising it. If position is fine and only volume is missing, a lift adds scars for nothing. Ask the surgeon to say which of the two you are before any sizes are discussed.
Why is this operation revised more often than others?
Because a lift tightens the skin envelope and an implant stretches it, so the surgeon is balancing two forces that work against each other on the same breast. That balance is harder to get exactly right than either operation alone.
Should it be done in one operation or two?
