Revision liposuction in Turkey

Revision liposuction is surgery to correct the result of a previous liposuction. The usual complaints are dents and ridges where too much fat was taken, areas that were left untouched, asymmetry between sides, and loose skin that the first operation exposed rather than caused.

It is a harder operation than the original, and that is the most useful thing this page can tell you. The tissue planes are scarred, the fat that remains is patchy rather than even, and the surgeon is working against someone else's plan. Expect improvement rather than a clean slate.

What actually goes wrong with liposuction

  • Over-resection

    Too much fat removed in one spot, leaving a dent or a groove. This is the hardest problem to fix, because the correction is usually adding fat back rather than taking more away.
  • Under-treatment

    Areas left with too much fat, or a fold at the edge of the treated zone. Comparatively straightforward, since it is more of the same operation done better.
  • Asymmetry

    One side different from the other. Common, and often the reason people seek revision even when neither side is objectively bad.
  • Skin that will not retract

    Not a liposuction error as such. If the skin had lost its elasticity, removing the fat underneath was always going to reveal it, and more liposuction will not tighten it.

Why revision is harder

  • Scar tissue. The plane the cannula moves through is no longer even, which makes a smooth result harder to achieve and the technique less forgiving.
  • What is left is uneven. The remaining fat sits in patches rather than a layer, so the surgeon is sculpting an irregular surface.
  • Adding is different from removing. Correcting a dent means fat grafting, and grafted fat partly reabsorbs, so more than one session is sometimes needed.
  • Expectations are higher and the margin is smaller. Someone who was unhappy the first time is understandably less tolerant the second, and there is less tissue to work with.

Timing: when a revision can be done

The single most common mistake is operating too early. Swelling, firmness and irregularity after liposuction take a long time to settle, and a proportion of what looks like a permanent problem at three months is not one at a year.

  • Wait until the result has settled. Surgeons generally want a substantial interval after the first operation, commonly a year, before judging what needs correcting.
  • Massage and compression first. Firmness and mild irregularity often improve with time and manual therapy, neither of which involves an operation.
  • Photograph the problem in the same conditions. Same light, same angle, same posture. A lot of perceived asymmetry is photographic.
  • Have the original operative note. What was done, where, and how much was removed. Without it the revising surgeon is guessing.

What the operation involves

  1. Assessment standing

    Irregularity is assessed upright and in movement, and marked while you stand. Dents that show only on contraction are marked differently from ones present at rest.
  2. Release, where needed

    Tethered scar tissue may need releasing before anything else, otherwise the dent stays no matter how much fat is added around it.
  3. Fat grafting for depressions

    Fat is harvested from elsewhere, processed and layered into the dent. Slightly more than the final volume is placed, because some reabsorbs.
  4. Careful liposuction for excess

    Where fat remains, it is removed with smaller cannulas and shallower passes than in a first operation.
  5. Compression

    Worn for weeks and, in revisions, often for longer than after the original surgery.

What drives the price in Turkey

  • How many areas and how complex. A single dent and a whole abdomen that needs re-contouring are very different operations.
  • Whether fat grafting is included. Adding fat means a donor site, more theatre time and a different recovery, and it is often quoted separately.
  • Whether more than one session is planned. Graft reabsorption means some patients need a second sitting, and you should know in advance whether that is included.
  • Anaesthetic and stay. Small revisions can be done under local anaesthetic; larger ones cannot.

If the loose skin is the real problem

Liposuction removes fat, not skin. If the surface is uneven because the skin has lost its elasticity, more liposuction will make it looser rather than smoother, and the operation that addresses it is a skin excision. Choosing a surgeon for a revision

  • Ask how often they do revisions. This is a subspecialty of a common operation, and volume in primary liposuction does not automatically mean skill in correcting one.
  • Ask to see revision results specifically. Before and after photographs of primary cases tell you nothing about how they handle scarred tissue.
  • Bring your operative note and photographs. Including photographs from before the first operation if you have them.
  • Expect honest limits. A good consultation for this will include a list of things that cannot be fully corrected.
