Hollywood smile in Turkey: prices and clinics

A Hollywood smile describes a result, not a treatment: teeth that are even, regular and lighter across the part of the smile that shows. There is no agreed clinical definition and no two clinics mean quite the same thing by it. What matters is which of the four treatments below your plan uses, because they differ enormously in how much of your own tooth they use up.

Tooth-coloured resin is shaped onto the tooth and set hard. Usually little or no tooth is removed, it can be repaired in the chair, and it can normally be taken off again. It stains over time and is the shortest-lived of the options.

Porcelain veneers

A thin ceramic facing bonded to the front of the tooth. Preparation is usually kept within enamel where the case allows, which is what makes bonding reliable. Measured by weight, preparing a front tooth removed 3 to 30 per cent of the crown of the tooth.1 Once prepared, the tooth always needs a restoration.

The tooth is reduced on every surface and capped. The same study measured 63 to 72 per cent of the crown of the tooth removed.1 Well suited to a tooth that is already broken down, heavily filled or root-treated. Less suited to a healthy tooth being treated for appearance alone.

How the four compare

Tissue loss leads the table, because it is the one part of this decision that cannot be changed later.

Neither option is better than the other in the abstract. A crown is the right answer for a tooth that has already lost a lot of structure, and the wrong answer for a sound one. Your dentist decides this after examining you, and the honest version of that conversation names what is being removed.

Start with the option that keeps most of your own tooth

Ordered by how much of the tooth each one uses. A careful clinician works down this list rather than starting at the end, and it is reasonable to ask why a step has been skipped in your case.

Straightening a tooth moves it without removing any of it, which is why a dentist may suggest aligners or braces before anything irreversible. Treating fewer teeth is a legitimate plan and often a better one: the number of teeth in a package is a commercial decision as much as a clinical one.

The problems people talk about in this category follow a pattern, and the pattern is over-treatment rather than geography. Careful clinics in Turkey avoid all of it, and careless clinics anywhere produce the same outcomes. Everything below can be settled by email before you travel.

What the evidence actually says

The most substantial evidence available is a British Dental Association survey of 1,000 United Kingdom dentists, published in July 2022.7 Of the 94 per cent who had seen patients returning from treatment abroad, 86 per cent had treated problems arising from it, and 87 per cent named crowns as the treatment most likely to need further work. On the cost of putting things right, 65 per cent said it came to at least £500, 51 per cent said more than £1,000, and one in five had seen cases exceeding £5,000.

What those dentists described is specific and avoidable. They reported teeth prepared more heavily than necessary, treatment carried out over untreated gum disease, and healthy teeth given extensive work in smile makeovers that would need a lifetime of maintenance. One described a young man given 21 full crowns for what was an orthodontic problem, correctable by moving the teeth rather than reducing them. Another described a patient who received 14 crowns and subsequently needed four root canal treatments.7

The trial smile is your last reversible decision

A well-run cosmetic case has a stage where you can see the proposed result before anything is removed. The dentist takes photographs, scans or impressions, and produces a design, either as a wax model or digitally. That design is then placed temporarily over your unprepared teeth as a trial smile, sometimes called a mock-up. You look at it, speak with it, and approve or change the shape and length.

Until that moment, nothing has happened to your teeth. After it, the enamel is gone. If a treatment plan has no trial smile stage, that is worth asking about directly, because its absence usually means the shape is being decided for you rather than with you.

Name the material and the laboratory

“Porcelain” is not a material, it is a family. Zirconia is strong and more opaque, which is why a solid zirconia restoration can look flat on a front tooth. Lithium disilicate, sold under the name E-max, is more translucent and is the usual choice where appearance matters most. Feldspathic porcelain can be made very thin and looks excellent, but it is the most technique-sensitive. In the pooled veneer data, lithium disilicate had significantly fewer technical and biological complications at 10 years than feldspathic.4

Which suits you depends on how much tooth is being kept, how hard you bite and where the tooth sits. What you can reasonably insist on is that the material is named precisely on your plan, and that you know which laboratory makes the work and whether it is in-house.

Questions worth sending before you pay a deposit

Copy these into an email. The answers should come back in writing, and any that do not is itself an answer.

• Are you proposing veneers or crowns, and will you confirm that tooth by tooth in writing?
• How much of each tooth will be removed?
• Would whitening, bonding or orthodontics achieve what I want with less tooth used up?
• Why this many teeth, and what changes if I treat fewer?
• Which material exactly, and which laboratory makes it?
• Will I see a trial smile before any tooth is prepared?
• How many appointments, over how many days, and is it one trip or two?
• Does the same dentist plan, prepare and fit the work?
• What does the warranty cover, what does it exclude, who issues it, and does claiming it mean flying back to Turkey at my own cost?
• What happens if a restoration debonds or fractures once I am home?
• What if a tooth needs root canal treatment after preparation, and who pays for it?
• Will I be given my treatment plan, radiographs, scans and material details to take home?

What has to be dealt with first

Cosmetic work goes on top of a healthy mouth, not instead of one. Active gum disease is treated before anything is placed, decay is dealt with, and failing old restorations are replaced. Bite problems and grinding are assessed, because a restoration that is fine in the mouth of someone who does not clench may not be in yours. If you are considering this while pregnant, elective work is usually deferred. Only an examination can tell you where you sit, and a clinic that wants to prepare teeth without checking any of it is skipping the part that determines whether the result lasts.

Restorations are replaced, not kept. The useful questions are how long they last, what needs attention first, and who looks after them when the clinic is in another country.

These are averages from clinical studies, mostly in patients followed by the practice that treated them. Your own result depends on how much tooth was left, how your bite loads the restorations, whether you grind, and how well the margins are cleaned. None of those are settled by the material.

The price to plan for is the lifetime one

If you are 30, plan for the work being replaced more than once. Each replacement means preparing the tooth again and using a little more of what is left, which is why the starting decision about how much to take away matters more than the opening price. A second course at today’s Turkish package prices is another €3,800 to €4,800, plus the travel, and a course done at home later costs whatever private treatment costs then.

What changes day to day

• Hard foods, ice, nuts and crusty bread are the usual causes of chipping. Bite with care rather than avoiding food altogether
• Do not open packaging or bite your nails with restored front teeth
• Coffee, tea, red wine and smoking stain composite and the margins of ceramic work
• Clean between the teeth around the margins. That is where decay starts on a crowned tooth
• Use a night guard if you grind, and replace it when it wears
• Keep up regular check-ups and hygiene appointments with a dentist at home
• Whitening will not lighten ceramic, so the untreated teeth can drift out of match over time

Who looks after it once you are home

Most things that need attention come up after the flight, which is the real difficulty with treatment abroad. The United Kingdom regulator is direct about the limits of its reach: the General Dental Council says it cannot guarantee that an equivalent regulator exists in another country or that standards are the same, and it has no authority to investigate complaints about treatment received abroad. It advises finding out whether the destination has a regulatory body and what its complaints process is, checking the dentist’s registration and insurance, and telling your own dentist about your plans so they know in case of later complications.8

In Turkey, clinics treating international patients require health tourism authorisation from the Ministry of Health, which is separate from the clinic’s operating licence and separate again from any accreditation it advertises. Complaints go to the Turkish authorities and, in practice, are far easier to pursue while you are still in the country than after you have left.

Whether treatment at home is available to repair private work done abroad, and on what terms, varies by country and by case. Confirm that locally before you travel rather than assuming it. Repairs are usually a private cost, and the British Dental Association survey found most remedial cases cost patients at least £500, with half exceeding £1,000.7

Seek urgent dental assessment for severe or increasing pain, swelling of the face or gum, a restoration that has come off and left a sharp or sensitive tooth, or a bite that has changed suddenly. Do not wait for a scheduled review.
