Butt reduction in Turkey

Three things create the outline: subcutaneous fat, the gluteal muscles underneath, and the pelvis and hip bones that both sit on. Liposuction addresses only the first. Muscle bulk and bone structure are not surgically reducible in any reasonable sense, and a plan that ignores which of the three is dominant will disappoint.

This is worth establishing before any price is discussed, because it is the difference between an operation that will work for you and one that will not. Ask to be examined standing and asked to contract, because that is how muscle and fat are told apart.

• Fatty, with good skin

  The situation liposuction suits. Volume comes out and the skin retracts reasonably, and this is the straightforward version of the operation.

• Fatty, with stretched skin

  Removing volume here can leave sagging. A lift may be needed as well, which changes the operation, the scar and the price.

• Muscular rather than fatty

  Liposuction will not change it. This is worth knowing before you travel rather than after you have paid for an operation that cannot address your shape.

• Wants a smaller waist, not a smaller bottom

  A common mismatch. If the complaint is really proportion, treating the waist and flanks is the operation, and reducing the buttock may make it worse.

   Examination, standing

   Skin quality, fat thickness by pinch, and whether the bulk is muscle. This is also where realistic limits are set, and it cannot be done from photographs.

   Deciding the areas

   The buttock rarely looks right treated in isolation. Flanks, outer thighs and the crease below the buttock are often part of the same plan.

   Anaesthetic

   Depending on the volume and the number of areas, local with sedation or general. Ask which and why, and what monitoring is used.

   The liposuction

   Fat is removed through small access points. In the buttock the surgeon has to leave enough behind, because over-removal here produces a flat, hollow shape that is difficult to correct.

   Compression and swelling

   A garment for weeks, and swelling that hides the result for longer than people expect. Ask how long the garment is worn and whether it is included.

   Months to judge

   Skin retraction and swelling both take months. The shape at six weeks is not the shape at six months, and revision decisions should wait.

• Anyone whose bulk is muscle. Liposuction removes fat. It does not reduce a muscular buttock and no honest surgeon will claim otherwise.
• Anyone with poor skin elasticity and no willingness to have a lift. Volume removal without skin removal will produce sag, and being told this in advance is the service.
• Anyone still losing weight. Wait until it is stable, because the shape you operate on will change underneath the result.
• Anyone using this to treat proportion. If the real wish is a defined waist, reducing the buttock can make the silhouette worse rather than better.
• Anyone expecting a BBL to be fully reversed. Grafted fat disperses through the tissue and removal is partial. Expect improvement, not undoing.

• How many areas are treated. The largest single variable, and the reason two quotes for the same operation can differ so much.
• Volume removed and technique. Different liposuction technologies are priced differently, though the surgeon matters more than the machine.
• Anaesthetic and setting. Local with sedation and general anaesthetic, day case or overnight, are different products.
• Whether a lift is added. Skin removal changes the operation entirely and should be quoted as its own line.
• Garments and aftercare. Compression, dressings, lymphatic massage if offered, and how many reviews are covered before you fly home.
• Whether more than one session is expected. Particularly when reducing previously grafted fat, where partial removal is the norm.
