IMSI, intracytoplasmic morphologically selected sperm injection, is ICSI with one extra step: before a sperm is injected into the egg, the embryologist examines it under a microscope at several times the magnification used for standard ICSI, looking for vacuoles and shape defects in the head that are invisible at normal power. The idea is that a sperm that looks better at high magnification carries better DNA and makes a better embryo. Everything else about the cycle, the stimulation, the egg collection, the culture and the transfer, is the same as any ICSI cycle.
The honest summary of the evidence is that IMSI has not been shown to produce more babies than ICSI. The most careful review of the trials found that it may make little or no difference to live birth or to clinical pregnancy, and that the studies are of low quality. It takes longer, it costs more, and it is sold in Turkey and elsewhere as a premium version of ICSI. It may still be reasonable to discuss after repeated ICSI failure with severely abnormal sperm, but as an option with uncertain benefit, not as a step up.
Standard ICSI selects sperm by motility and gross shape at ordinary magnification. IMSI examines the head at much higher magnification, using a technique called motile sperm organelle morphology examination, and rejects sperm with large vacuoles or abnormal head shape. The rationale is plausible: vacuoles are associated with DNA fragmentation, and DNA damage is associated with poorer embryos and higher miscarriage.
Plausible is not proven. The randomised trials comparing IMSI with ICSI, pooled in a Cochrane review, do not show a clear benefit in live birth, and the quality of the evidence is low. Some trials in specific groups, men with severe teratozoospermia or couples with repeated implantation failure, suggest a possible effect, and others do not. That is where the discussion belongs: with a specific problem, a specific history, and an honest statement that the benefit is uncertain.

A semen analysis showing severely abnormal shape is a finding, not a diagnosis. The causes that can be found and sometimes treated include varicocele, infection, hormonal disorders, obstruction, medication and lifestyle factors, and genetic causes that matter for the child as well as for the treatment. A urologist with an interest in male fertility should see the man before any laboratory technique is chosen.
Sperm DNA fragmentation testing is sometimes offered alongside IMSI. It measures a real thing, but its value in deciding treatment is itself uncertain, and a high result does not mean IMSI is the answer. Treat both as part of a discussion with a specialist, not as items on a menu.
