• Over-plucking that never recovered

  Years of plucking can permanently damage follicles. This is the commonest reason for the procedure and it responds well.

• Scarring or injury

  A scar through the brow, or loss after burns or surgery, where nothing will regrow on its own. Grafting into scar tissue is harder and survival is lower, which the surgeon should say.

• Naturally sparse brows

  Some people simply have thin brows. This works, provided expectations match what the donor hair will do.

• Not for active hair loss conditions

  Alopecia areata, frontal fibrosing alopecia and thyroid disease can all cause brow loss. Transplanting into an active condition wastes the grafts, so the cause must be assessed first.

   Design

   The brow shape is drawn with you sitting upright and awake, and you should agree it before anything starts. This is the step that decides whether you like the result.

   Donor extraction

   Follicles are taken one at a time, usually from the back of the scalp above the neck, under local anaesthetic. Single-hair units are selected.

   Angling the sites

   Recipient sites are made at a very shallow angle so the hair lies flat. This is the technical heart of the procedure.

   Placement

   Each follicle is placed individually, following the direction hairs grow in different parts of the brow, which changes from the inner end to the tail.

   Same-day discharge

   You leave with small crusts along the brow that settle over about a week.

• How many grafts. The main factor. Filling a gap needs far fewer than rebuilding two absent brows.
• One brow or both. Matching a single brow to an existing one is its own challenge and is sometimes priced differently.
• Whether it is scar tissue. Lower survival means a second session is more likely, and you should ask whether that is included.
• Who performs it. Whether the surgeon designs and places, or technicians do most of the work. On brows, where angle is everything, this matters more than usual.
