Ethnic rhinoplasty in Turkey

• Skin thickness

  Thicker skin hides the refinement made underneath and holds swelling far longer. It is the single biggest limit on how much definition a tip can show, and it cannot be changed.

• Cartilage strength

  Softer, weaker lower cartilages give less support. Reducing them without adding support is how tips collapse and airways narrow, which is why grafting is common in these noses.

• Base width and nostril shape

  A wider alar base is often part of the concern. Narrowing it means external incisions at the nostril creases, which leave small but permanent scars.

• Dorsal height and tip projection

  A lower bridge or less projected tip may need building up rather than reducing, which is the opposite of what a classical reduction rhinoplasty does.

• Swelling lasts much longer. Where a thin-skinned nose settles over months, a thick-skinned tip can stay swollen well beyond a year.
• Definition is limited. Skin drapes over the framework. No technique produces sharp definition through thick skin, and a surgeon promising it is overselling.
• Judging early leads to bad decisions. Asking for a revision at six months on a thick-skinned nose usually means operating on swelling.
• Support matters more, not less. A heavier skin envelope needs a stronger framework underneath to hold the shape, which is why grafts are used.

   Assessment

   Examination inside and out, an honest read on skin thickness and cartilage strength, and a discussion of what you actually want changed.

   Planning the grafts

   Decided in advance: septum if usable, otherwise ear or rib. This should be settled before the day, not in theatre.

   Open approach, usually

   A small incision across the columella lets the surgeon place and fix grafts accurately, which matters more when structure is being added.

   Reshaping and support

   The bridge is built or refined, the tip is supported and shaped, and the base is narrowed only if that was agreed.

   Splint and settling

   An external splint for around a week, then a long settling period that is longer than average where the skin is thick.

• How much structure is added. Building a bridge and supporting a tip is a longer operation than reducing one.
• The graft source. Rib harvesting adds theatre time and recovery and is priced above septal or ear cartilage.
• Whether the base is narrowed. An extra step with its own technique and its own scars.
• Whether breathing is corrected too. Functional work alongside the cosmetic change adds to the operation.
