Turkey has a large, competitive private cosmetic surgery sector concentrated in Istanbul, Izmir and Antalya, and a long-established medical travel industry built around it. For fat grafting specifically, that concentration matters more than it does for simpler procedures, because the result depends heavily on how the surgeon harvests, processes and layers the fat, and volume of practice is what builds that skill.
Three things are worth checking before the price:
Fat transfer is priced per case rather than per procedure, which is why quotes vary so much between clinics for what looks like the same operation. What moves the number:
When you compare a Turkish quote with one from home, compare like with like: ask both for the surgeon's fee, the facility fee, the anaesthetist's fee, the garments and the follow-up, itemised.

Fat grafting needs a donor site. You need enough fat somewhere on the body to harvest, which is why very lean patients are sometimes told they are not suitable, or are offered a smaller result than they wanted.
Beyond that, surgeons generally look for:
Facial fat transfer, also called facial lipofilling, replaces volume that has been lost from the midface, temples, cheeks, jawline and lips. It is the form of fat grafting Turkish clinics most commonly advertise, and the one this page is built around.
The fat is usually harvested from the abdomen or inner thighs, processed into fine parcels, and placed in small amounts across several depths so that each parcel sits close to a blood supply. Surgeons often distinguish:
Facial work is frequently done under local anaesthesia with sedation, and it is the reason many patients are back to non-physical work within about a week. Expect to look swollen and overfilled at first. That is deliberate, because a proportion of the graft will not survive.
Under-eye fat transfer, sometimes called tear trough fat grafting, softens the hollow between the lower eyelid and the cheek. It is a distinct operation from lower eyelid surgery, and the two are sometimes combined.
It is also the most technically demanding place to put fat. The skin is thin, the tissue plane is shallow, and over-correction shows. Ask specifically:
If your hollowing is mild, ask whether a filler or a lower blepharoplasty would suit you better. A surgeon who will talk you out of an operation is a good sign.

Fat transfer is a single operation done in one sitting. These are its stages.
The surgeon assesses the donor and recipient sites, agrees the plan with you, and marks the harvest areas while you are standing.
Local anaesthesia with sedation is common for facial work. General anaesthesia is usual when larger volumes are harvested or several areas are treated.
A tumescent solution is infiltrated into the donor site to reduce bleeding and bruising, then the fat is drawn out through a fine cannula at low pressure. Gentle handling here is what keeps the fat cells viable.
The aspirate is separated from blood, oil and fluid by centrifugation, filtration or decanting, leaving usable fat parcels.
The fat is injected in many small passes at different depths rather than into a single pocket, because each parcel needs to sit close enough to living tissue to pick up a blood supply.
Recovery has two clocks running: the donor site, which behaves like liposuction, and the recipient site, which is about swelling settling and the graft taking.
Follow the clinic's own instructions on sleeping position, pressure on the grafted area, and when to resume exercise. For facial and under-eye grafts in particular, avoiding pressure on the treated area matters more than most patients expect.
Fat transfer is surgery and carries real risk. A clinic that does not discuss these with you is telling you something.
Fat that survives the first months and establishes a blood supply behaves like the tissue it came from, so the result is long-lasting. It is not fixed for life: the grafted fat responds to weight change, and the face continues to age around it.
Judge the result at the point your surgeon tells you to, not before. Ask to see before and after photographs of the specific area you are having treated, taken at a consistent distance and lighting, at a stated interval after surgery.
