Overview
Cystectomy is a medical operation that includes total or partial bladder removal, and in some cases, the operation includes the construction of a new bladder, neobladder. Cystectomy bladder surgery is performed for a solution to bladder problems like bladder cancer, UPJ obstruction (Ureteropelvic Junction Obstruction) just as to some neurological disorders.
Also, cystectomy procedure is made for ovarian cyst removal, but they don’t have much in common. In this article, we mention cystectomy as a bladder removal.
Preparation
If you have other medical problems, first you should inform your doctor. Then, a CT scan and different tests may require. Eating something before the operation is forbidden just as drinking anything hours before the operation. In some cases, mechanical bowel may be performed, but there are alternatives, and some doctors have doubts about this procedure has positive effects.
How It Is Performed
Cystectomy operation may be performed in two pays; partial or total bladder removal. In partial removal, doctors remove only scar tissue on the bladder and stitch up. In total bladder removal, the whole organ is removed. The second type of cystectomy generally performed if a patient has bladder cancer. During the operation, other organs and tissues may be removed with bladder as well. Cystectomy may be performed as open surgery through an incision on the abdomen. If partial bladder removal is performed, doctors may perform laparoscopic surgery, and it can be performed with a few small incisions. After the whole bladder removal, doctors have a few options to choose for the reorganization of a urinating system according to patient’s needs. A way to reorganize urinating system that becoming popular is called neobladder, a new bladder constituted from a piece of intestine. Another popular way is the ileal conduit or cystostomy. Patients can urinate with a created stoma and catheter.
Recovery
The recovery period after cystectomy operation based on the type and extent of one’s bladder removal. Partial cystectomy’s healing process more rapid than radical cystectomy operation, due to chemotherapy and radiation therapy which needed after radical cystectomy. Generally, it takes to rest in the hospital for about a week. While you rest in hospital you’ll take strong painkillers and doctors will focus on your healing process. After this week, patients can leave the hospital. When you get into your normal function, doctors recommend you to walk and exercise, in case of neobladder creation, it will be more important to increase the time of urine kept in neobladder. The recovery process may take 6-8 weeks for the whole period of cystectomy.
Risks
Partial and radical cystectomy has some possible risks. Bladder removal may cause urine or stool leakages and kidney problems. Also, stoma problems may be seen by some patients after cystostomy. Nevertheless, these risks are uncommon.
Side Effects
Patients who undergo radical cystectomy which using neobladder or a bladder bag after cystostomy may experience some difficulties that will be pass in time, and it will become more and more acceptable.
Success Rates
This operation’s success rate is based on the problem and the extent of bladder removal. The success rate depends on the stages and kinds of bladder cancer, changing from %50 to %90.

Long waiting times in UK…
Because of long waiting times in UK hospitals due to Covid19, I hit upon the idea of having my bilateral hernia surgery done while on a 2-week holiday in Antalya. After making inquiries through Yasmin, I had a short but comprehensive consultation with an eminent surgeon Prof Alihan Gürkan and my operation was scheduled for a day halfway through my stay in Antalya. It went well with keyhole surgery, I stayed in overnight and was discharged the next day, which gave me 6 days to recover before my flight home to Scotland. Yasmin kept in touch throughout, making sure I had transport to and from the hospital, and a special thanks to Fatma, my mentor and guide at the Memorial Hospital on my day of tests and examinations. I telephoned my surgeon a few days after the operation for advice on removing my plasters and he was most helpful and reassuring about my complete recovery. I was not too fond of the breakfast, but considered it as another form of medicine! My flight home was amazingly comfortable without painkillers, although they were definitely needed for a few days after my operation. In conclusion, I would be happy to have other treatment like this, though in a nice sort of way I hope never to set eyes on the Memorial Hospital again. 🙂
